How loud does a noise have to be to cause hearing loss?

Sound pressure is measured in decibels, with a regular conversation measuring at 60 dB and a pair of earphones at max volume at over 100 dB. Noises over 100 dB can cause damage after constant exposure and noises at 120 dB (the sound of a distant lightning strike) can cause immediate damage. Damage is permanent or temporary and can affect one ear or both ears.
